Simple Checklist Leads to 82% Drop in Mental Health Patient Suicides

A new study shows a sharp decline in suicides at Veterans Affairs inpatient mental health units from 2000 to 2015, thanks to the Mental Health Environment of Care Checklist.

The checklist, introduced into the VA in 2007 and used by more than 150 VA hospitals nationwide, guides staff in eliminating physical hazards at the mental health inpatient units which raise the risk of patient suicide or self-harm.

The researchers found a sustained reduction in inpatient suicides during the last seven years in the study period, with none occurring each of the last three years.

 The checklist focuses on architectural and other physical-environment changes. Earlier analyses had shown that most attempted or completed suicides on inpatient units were linked to these hazards–for example, a hook or other anchor point that could be using for hanging.

Someone did good work here. All the negative stories about the VA aside, they should get kudos for this idea.

Now how can we expand on that?

Is every hospital using these type of checklists?

There is a great book “Checklist Manefesto” https://www.amazon.com/dp/B0030V0PEW/ref=dp-kindle-redirect?_encoding=UTF8&btkr=1 that I bought for all my subordinates at the time.

Hope everyone can prevent suicides and other premature loss of lives. I do it by driving carefully and saying a prayer for the “anal sphincters” who drive suicidally.

Brian Patrick Fagan, 76, of Bonita Springs, passed away Saturday, December 3, 2016 after a valiant battle with illness.

He was born April 6, 1940 in Orange, NJ. He received his BA Degree from Seton Hall University in South Orange, NJ; his MA in Education from Manhattan College; his MSW from New York University; his MA in Psychology from New School for Social Research in New York and his Doctorate in Psychology from Pace University in New York.

Dr. Fagan began his life of service as a priest in the Roman Catholic Archdiocese of Newark, NJ and continued as a social worker for the Division of Youth and Family Services in NJ. He served as a psychologist on the Child Study Team for the School District of Chatham in Chatham, NJ. He also practiced adolescent and adult psychology privately.

After retiring to Florida, Brian served as a Guardian Ad Litem and as a mentor for high school students. He was enthusiastic about everything he did and loved reading, spirited conversations, the arts, Seton Hall Basketball and Notre Dame football. He was gifted with Irish wit and a deep commitment to civic engagement as well as compassion for the less fortunate among us.

Brian is survived by his wife Leona and his nieces and nephews of whom he was very fond.

A Memorial Mass will be celebrated at 9:30 AM on Saturday, December 17, 2016 at St. Leo Catholic Church, 28290 Beaumont Road, Bonita Springs, FL 34134.

William Gaillard Farrell, 86, of Hendersonville, NC passed away on December 10, 2016 at Elizabeth House in Hendersonville.

He was born March 29, 1930 in New York, NY.

Bill graduated from St. Elizabeth Catholic School, Cardinal Hayes High School and Manhattan College, all in New York City. He enlisted in the Navy while attending Manhattan College and ultimately was Commissioned through the Naval Reserve Officer Candidate program after he graduated in 1952.

He served on Active Duty during the Korean War including tours aboard two aircraft carriers, the USS Salerno Bay and the USS Leyte. Following his Active Duty, and a return to the construction field in New York City which included overseeing the major redevelopment of City College of NY as Director of Campus Planning, and Development and Facilities Planning at Brooklyn College, he remained in the Naval Reserve, ultimately transferring to the Navy Civil Engineer Corps.

During the next 25 years of military service, Bill served with distinction in the Reserve Naval Construction Forces in key leadership positions supporting the Seabees, following a tradition set by his father. His most notable tours include assignments as the initial Inspector General of the First Reserve Naval Construction Brigade; Commander, 21st Naval Construction Regiment; and Commanding Officer, Naval Mobile Construction Battalion Thirteen. He retired as Captain after a thirty year career.

Bill and his wife, Madeline, retired to Hendersonville in 1991.

Bill was a proud Eagle Scout and participated as the Senior Patrol Leader of all the scouts from New York City at the World Scout Jamboree in Moisson, France in August, 1947.

Bill was very involved in church, community and civic activities, both in New York City and Hendersonville. This included in New York serving as a Boy Scout adult leader, involvement in the St. Luke's parish council and serving as an organizer and the announcer for the parish bazaar; and in Hendersonville many volunteer activities for Immaculate Conception parish, including Vice Chairman of the committee which built the current church, the Gem and Mineral Society, the Henderson County Sheriff's Department, the Solid Waste Advisory Committee, the Board of Equalization and Review, the Planning Board and the Republican Club, Member of the Korean War Veterans, Member of the Military Officers' Association of America (MOAA) and involvement in several veteran's activities, which included accompanying veterans on Honor Flights to the World War II memorial. Bill also served on the Blue Ridge Community College Board of Trustees.

He was the son of the late William R. Farrell and Abby (nee Gaillard) Farrell. In addition to his parents, he was predeceased by an older infant sister, an infant grandson, an infant great granddaughter and son-in-law.

Bill is survived by his wife of 64 years, Madeline C. Farrell (nee Wenz), his nine children, Madeline Franki, William (Marleny), Gregory (Donna), Dennis, Philip (Maureen), Kevin (Ada), Raymond (Karen), Regina Bellantuono (Robert), Donald (Marisa), his twenty-seven grandchildren, his fourteen great grandchildren, his sister Loretta Dente (Anthony), his brother Philip (Julie), his sisters-in-law Ernestine Wenz and Rose Marie Marron, and many nieces, nephews and cousins.

Bill was a loving son, husband, father, grandfather and great grandfather totally devoted to his family, God, country and friends.

The family will receive friends on Friday, December 16, 2016 from 6-8pm at Forest Lawn Funeral Home, 538 Tracy Grove Road, Hendersonville, NC with a prayer service beginning at 6pm. A funeral mass in celebration of his life will be held on Saturday, December 17th in the main sanctuary of Immaculate Conception Church in Hendersonville. Military honors will be rendered in the church following the mass. Interment will be held at Arlington National Cemetery, Washington, DC at a date to be determined.

Margaret Cooney and Matthew Sorensen, both of Virginia Beach, Va., are pleased to announce their engagement.

The bride-to-be, daughter of Timothy and Wendy Cooney, of Newton, graduated from Pope John XXIII Regional High School in 2010, and from Manhattan College in 2014. She is a real estate paralegal.

The groom-to-be, son of Kristin and David Sorenson, of Cranston, R.I., graduated from LaSalle Academy in 2010, and from Manhattan Colllege. He is a technical engineer.

The couple has planned a wedding that will take place on Sept. 23, 2017, in Virginia Beach, Va.

Bronx, NY (December 9, 2016) - Mount St. Michael Academy has appointed Thomas A. McCarthy to serve as Vice President of Institutional Advancement. McCarthy comes to The Mount from Manhattan College where he held the position of Director of Alumni Relations, a role he assumed in 2011. Prior to his appointment, McCarthy served as Assistant Director for Annual Giving, and focused on sustaining and recruiting donors and prospects, while connecting and cultivating alumni at a variety of events.

As Director of Alumni Relations at Manhattan College, McCarthy played a crucial role in leading alumni relations into a new era, including enhancing alumni communication, maintaining and building relationships with young and senior alumni, planning and organizing a variety of events, and much more. His alumni and development experience were instrumental factors in guiding Manhattan College's alumni office and broader alumni community into meeting the strategic goals of the institution.

After graduating from Manhattan College with a Bachelor of Science in secondary education and a concentration in mathematics in 2006, he initially worked as a teacher at a Catholic high school in Westchester County before returning to his alma mater. McCarthy's attraction to Catholic education continues; he begins his role at The Mount on December 12, 2016.

McCarthy, originally a native of Riverdale, N.Y., is a resident of Yorktown Heights, N.Y. He is the son of Thomas McCarthy, Sr., MSM Class of 1964. He completed a master's in general counseling in 2010, and is currently working toward a Ph.D. in Catholic and non-public school leadership at Fordham University.

After serious thought and consideration, I've recently accepted the position of vice president for advancement at Mount St. Michael Academy in the Bronx and will be leaving Manhattan College.

Given the time of year, and the immediate needs of Mount St. Michael, I've already started to transition into my new role and will officially begin this Monday, Dec. 12. This position is an excellent opportunity for me with responsibilities in development, alumni relations and communications for the school, which has been a staple in the Bronx for 90 years and currently serves grades 6-12 - it also happens to be the high school my father graduated from some 50+ years ago. After serving the College for a decade (almost to the day) it's very hard to leave but I know this is a terrific move for me personally and professionally.

The College, and our vice president, Tom Mauriello, have been extremely supportive of this opportunity and I'm fortunate to have my Manhattan colleagues continue to remain available to me as mentors and resources. I will be seeing through some obligations to the Alumni office at Manhattan over the next few weeks to help ease the transition. Of course, should you have any questions or need help, our manager of alumni relations, Kelly Douglas, will be on hand to hold down the fort until the College is able to fill the current vacancies.

I've often said that I'm the "luckiest guy on campus" being able to work as an alum at my alma mater, and meet so many great Jaspers throughout the country. It's been a pleasure and privilege to work with you in making Manhattan's alumni presence stronger and I'm proud of all the activities we've put forward. We've accomplished so much and created some lasting memories that I know will stay with me for a long time.

Good Evening All,

 
I have heard from Bob Mattis regarding his very good friend Sue. Sue has been in our prayers as she went in for surgery on December 6th at Mass. General to remove a Tumor from her Salivary Gland and Lymph Nodes. She did not know to what extent her Lungs were involved.
 
Sue found out that the Cancer is in her lungs and she will need treatment. Sue is also having surgery on Wednesday, December 21st, on her Salivary Glands. Please keep the volume up for Sue as she prepares for surgery. We pray that it is successful in removing the Cancer and that her treatments are successful in arresting and eliminating her Cancer in her Lungs. We continue to pray that God Guides the Surgeon's hand. Let our prayers be felt by this good woman and  that God's Grace Embrace her during this trying time. We raise the volume for Sue, her Family and dear friends. Thank you. God Bless.

Walter P. Fitzgerald died peacefully on December 10, 2016 after a long illness. He was 83. A longtime resident of Rye and then Larchmont, Walter was born on July 4, 1933 in New York City, the son of Walter and Rose (Brady) Fitzgerald. After attending Manhattan Prep, Walter served with the US Army in Korea from 1953-1955. He later graduated from Manhattan College and Columbia University with degrees in Economics and went on to a career in banking.

Walter was predeceased in 1998 by his wife, Marilyn (D'Arco) Fitzgerald and in 2003 by his older brother Aloysius Fitzgerald of the Congregation of Christian Brothers. He is survived by his younger brother John Fitzgerald of Chicago; his daughters Tracey Sullivan (Howard) of Rye, Joan Clarke (Christopher) of Larchmont and Pamela Fitzgerald of South Norwalk, CT; his six grandchildren Caroline, Stephen and Elizabeth Clarke and Charles, Allison and Scott Sullivan; and his cousin Theodore Morrissey of Burlington, NC.

A wake will be held Friday, December 16, from 6-8 pm at Graham's Funeral Home in Rye. A Mass of Christian Burial will be held Saturday, December 17 at 10 am at Church of St. Augustine's in Larchmont. Internment will follow at Greenwood Union Cemetery in Rye.

He was served in the US Navy from 1946 to 1947.

A vigil will be held Monday 1219 at Saddleback Chapel in Tustin 5-9 p.m. Rosary at 7 p.m. Funeral Mass will be held at La Purisima Catholic Church Tuesday 1220 at 10 a.m. with burial to follow at Holy Sepulcher Cemetery.

Alexandra Simony has been promoted from project coordinator to associate manager, Industry Relations. In her new role, Simonfay will plan and execute organization meetings, manage events for APPA’s industry-related initiatives and manage contacts and secure event space. She will also be responsible for overseeing the development of the Global Pet Expo Show Directory and assist with exhibitor registration and buyer qualification at the Show. Simonfay earned a bachelor’s degree at Manhattan College and a Master of Arts at Marist College. She recently earned a Certified in Exhibition Management™ (CEM) designation, and is a member of the IAEE and the American Society of Association Executives (ASAE). She was also recently named among IAEE’s “20 Under 30.”

Obituary

Captain Joseph Sutherland Myers, MC, USN (Ret.), 85, passed away peacefully surrounded by family on December 15, 2016 in New Smyrna Beach, FL.

He was preceded in death by his beloved wife of 55 years, Patricia Marie Myers (nee McKenna), he leaves son Joseph S. Myers, Jr. and Diane of Port Orange, FL, daughter Barbara Davis and Christopher of Oviedo, FL, son Christopher and Christina of Fairfax, VA, son James and Tina of New Smyrna Beach, FL, eleven grandchildren, and step-brother John Ranalli, of Wilmington, NC. He was also preceded in death by his parents and his brother, Thomas.

Born on August 24, 1931 and raised in Albany, NY, the son of Joseph P. and Dolores Myers (nee Sutherland), he was Valedictorian of his 8th grade class at West Albany School No. 19. From there, he attended Christian Brothers Academy in Albany, NY for high school where he continued to excel academically graduating third in the Class of 1949 and achieving the rank of Cadet Major. After high school graduation, he attended Manhattan College in Riverdale, NY where he completed his pre-med studies and graduated with honors in 1953. He then attended Marquette University Medical School (now the Medical College of Wisconsin), in Milwaukee, WI and graduated in 1957.

After completing a residency and a preceptorship in general surgery, he was commissioned in the U. S. Navy, Medical Corps, and served from 1963 to 1987. He spent those years moving around the country and the world including Argentia, Newfoundland, Guam, and many other U.S. station locations. He served as Chief of Surgery at several Naval Hospitals, and it was his privilege to care for many injured Soldiers, Marines, and Sailors from the Vietnam War while stationed at U.S. Naval Hospital, Guam.

His love of the warm, Florida weather contributed to his decision to move to Daytona Beach, Florida upon his retirement from the Navy in 1987. He continued to practice medicine until his full retirement in 1999.

He was a devout Catholic, a humble and skilled surgeon, a scholar of American History particularly the Civil War Era, a patriot, a pro-life advocate, and a family man. He was committed to providing his children with a quality education and to passing on the Catholic faith to them. He will be greatly missed.

In lieu of flowers, donations may be made to the Pregnancy Crisis Center of Daytona Beach. Visitation will take place from 4:00 PM to 6:00 PM on December 18, 2016 at Volusia Memorial Funeral Home, Port Orange, FL. A Mass of Christian Burial will be celebrated by his long time pastor and friend the Reverend Raymond O'Leary at 8:30 AM on December 19, 2016 at Our Lady of Hope Catholic Church, 4675 S Clyde Morris Blvd., Port Orange, FL. Condolences can be shared with the family by visiting www.volusiamemorialfuneral.com.
